Carpal Tunnel Syndrome

Carpal tunnel syndrome (CTS) is a condition that affects the hand and wrist, caused by compression of the median nerve, which runs through the carpal tunnel in the wrist. The carpal tunnel is a narrow passageway on the palm side of the wrist, formed by bones and ligaments. The median nerve, as well as tendons, run through the carpal tunnel and provide sensation and movement to the thumb, index, middle, and part of the ring fingers.

Symptoms and Causes

Symptoms of CTS include:

  • Numbness, tingling, and burning sensation in the fingers, especially the thumb, index, and middle fingers
  • Weakness in the hand and fingers
  • Pins and needles sensation in fingers
  • Pain that radiates up the arm
  • Loss of grip strength
  • Symptoms tend to be worse at night and can cause difficulty sleeping

Repetitive motions that put stress on the wrist, such as typing on a computer, using a mouse, or performing assembly line work can lead to CTS.

Trauma in the form of fractures or dislocations of the wrist can cause swelling, which can compress the median nerve. Rheumatoid arthritis, gout, or other inflammatory conditions can cause inflammation in the wrist and compress the median nerve. Hormonal changes during pregnancy can cause fluid retention and swell in the wrist, which can compress the median nerve. Being overweight or obese can put pressure on the median nerve. It is important to note that most cases of CTS are caused by a combination of factors, including the combination of genetics, lifestyle, work environment, and medical conditions.

Treatment options for CTS include splinting the wrist to immobilize it and reduce stress on the median nerve, nonsteroidal anti-inflammatory drugs (NSAIDs) to reduce inflammation and pain, steroid injections to reduce inflammation in the carpal tunnel, physical therapy to improve the strength and flexibility of the hand and wrist and surgery (carpal tunnel release) if symptoms are severe or do not improve with other treatments.
